Our goal is to help the patient return to a more functional and active lifestyle as quickly as possible. The therapist or social worker will work in unison with the physician to set up a care plan that will be most beneficial to the patient to increase their overall level of functional mobility and safety.
Physical and occupational therapy can improve strength, flexibility, and reduce pain. They can also improve balance, stamina, coordination, postural alignment, and decrease fall risks. This includes, but is not limited to, gait training and functional daily activities such as getting in and out of bed or a chair, the car, the bathtub and shower, and going up or down stairs in a safe manner. Physical and occupational therapists can also use modalities to reduce pain.

Speech therapy can improve speech and language skills as well as cognition and memory.

It can also improve eating and swallowing problems and disorders.
Social workers can assist you or your loved one with long term planning, community resources such as meals on wheels, the Advantage program application process, alternate living arrangements and short term therapy.
Dieticians can develop a proper nutritional plan for your patients as well as educate them on diets. This can help prevent rehospitalizations especially in our diabetic and CHF clients.
The therapist or social worker will work hand in hand with your home health or hospice agency to ensure all of your home health needs are met. Our caring and compassionate therapists and social workers will be there to answer all of your questions and concerns to help accelerate your recovery.
